Hospice RN Case Manager
Are you or someone you know, looking for a mission-centered nursing position? If so, Franciscan Home Care and Hospice Care, a faith-based home health care agency, Meriden, is seeking a Registered Nurse to work full-time its hospice care program.
The RN Hospice Case Manager is responsible for overseeing the care of patients who are near end of life. They work with an interdisciplinary team of medical professionals to ensure hospice patients receive high-quality palliative care, including physical, emotional, and spiritual support. Hospice case managers also help families navigate during this difficult time by providing care and support.
Hospice case managers have a wide range of responsibilities, which may include:
- Meeting regularly with patients and their families to monitor the patient’s physical, emotional, and spiritual needs
- Assessing each patient’s social and medical history to develop plans of care that meet their needs
- Working with physicians, nurses, and other medical professionals to coordinate care and treatment plans for each patient
- Maintaining records of patient care plans, physician orders, medication administration, and other information about a patient’s condition
- Coordinating care with other medical providers and social services agencies in order to meet patients’ needs
