Franciscan Home Care and Hospice Care is pleased to announce receipt of a grant from The James H. Napier Foundation, a local private foundation serving Meriden and Wallingford communities, to help promote Independence for the Elderly in the two towns.
Sylvia Sanguano, Social Case Worker
Through this grant, Silvia Sanquano, Social Case Worker, will help provide assistance to low-income and minority elderly to achieve and/or maintain maximum independence and productivity while living in their homes as long as possible. This is accomplished by identifying individuals, their social, familial and emotional needs and assisting them in addressing those needs.
