The Hospice RN Case Manager is responsible for overseeing the care of patients who are near end of life. They work with an interdisciplinary team of medical professionals to ensure hospice patients receive high-quality palliative care, including physical, emotional, and spiritual support. Hospice case managers also help families navigate during this difficult time by providing care and support.
Qualifications: RN License, Driver’s License
